What is a tuberous breast - and how can you recognise it?

A tuberous breast is a developmental disorder of the breast gland and connective tissue. The cause lies in a ring-shaped band of connective tissue at the base of the breast. During puberty, this band prevents the glandular tissue from developing evenly into all quadrants. As a result, the lower part of the breast remains underdeveloped.

A typical sign of a tuberous breast is therefore a narrow breast base with little fullness in the lower area. This makes the contour appear more pointed than round, the inframammary fold often sits too high, and the areola can appear enlarged or slightly bulging. The malformation can affect one or both breasts and often stands out through asymmetry.

Grading of tuberous breast severity

Depending on severity, a tuberous breast can be classified into four grades:

  • Grade 1: Usually only one lower breast quadrant is affected; the rest of the breast shape appears largely normal.
  • Grade 2: The entire lower half of the breast is underdeveloped. Asymmetry is common, and the nipple may protrude slightly forward.
  • Grade 3: The breast is noticeably smaller and shows a degree of ptosis (sagging); the nipple is visibly prominent.
  • Grade 4: The malformation affects the entire breast. It hangs downward in a tube-like shape; the nipple and areola are significantly enlarged and often bulging.

Tuberous Breast or Sagging Breast?

Many women confuse a tuberous breast with a sagging breast, although these are two very different starting situations. With a tuberous breast, the lower part of the breast is underdeveloped, whereas a sagging breast is not a malformation but a drop in the breast caused by a loss of skin elasticity. Here, the breast tissue lacks firmness and sometimes volume.

Who should consider treatment, and from what age?

Treating a tuberous breast makes sense whenever you feel uncomfortable with the shape of your breast - regardless of whether the burden is primarily psychological or also functional. A tuberous breast can be accompanied by underdevelopment of the glandular tissue, which can limit milk production. Breastfeeding difficulties can occur especially with grade 4.
